Make a request for a reservation by using the forms below.  Until the forms are signed and turned in at the church for conformation and approval, your date is not reserved. All three pages need to be returned.  

If you are interested in renting the Family Life Center, download and print the forms below. Fill them out and submit them to the church either in person or mail to the Kirkpatrick Church of Christ, 5154 Columbus-Sandusky Rd. N. Marion, Ohio 43302-9719.

Once forms are received you will be informed whether your desired date for the rental is available. Thank you.
